You will be contributing 1.75% of your basic salary while your employer will complement it with 5% of your basic salary
You (the principal enrollee), your spouse (wife or husband) and 4 biological children below 18 years of age
They are automatically removed as dependents. You may register them under our vital contributor plan at Fifteen thousand Naira (#15,000) per annum or they join the TISHIP programme if they are students of any of our State-owned Tertiary Institutions in the State.
The waiting period is 42-90days from the date of enrolment.
Pay additional premium of #15,000 into the KGSHIA bank account for them to be enrolled.

Yes you can change your health care provider within 6 months due to relocation but after 6months if the reason for change is due to complaint about the health care provider to allow KGSHIA resolve all grievances relating to your provider.
You can replace your dependents for any of the reasons listed below.
a. When the dependents are above 18 years of age.
b. When you want to add a new child or replace a dependent with another.
c. In cases of death, divorce etc.
It is free of charge.
Yes, but it has to be within Kogi State.
Yes, both of you must be registered because deduction is from both salaries.
Yes, because your premium will still be deducted from the salary.
No, your premium would not be refunded to you because health insurance involves pooling of funds and sharing the risk.
